How to Prepare for Your Consultation
1. Choose How We Connect
Our initial 30-minute encounter is conducted via video (FaceTime, Skype, or Zoom). I will call you directly at your selected time.
2. Have These Ready Before We Call:
- A valid government-issued Photo ID.
- Your preferred pharmacy name and address so I can instantly transmit your prescriptions.
3. After Booking your Appointment, you will receive links to:
- A medical history form to complete and submit.
- An Asthma Control Test to complete and submit
4. Serving patients located in CA, AZ, NV, CO, NE, TX, TN, AL, GA, FL. Appointments are conducted completely online.
5. Need a Prescription Refill?
